Allied Health Applications for Initial Licensure

Things to Know Before You Begin

  1. This online initial license application is available for the following Allied Health practitioner types:
    • Athletic Trainer
    • Naturopathic Doctor
    • Genetic Counselor and Supervised Genetic Counselor   What is the difference?
    • Nuclear Medicine Technologist

  2. Did you get fingerprinted? A criminal history records check is required by law in Maryland. Completing your background check BEFORE you begin will expedite the licensure process. Click CHRC Instructions for details.

  3. The fee for an initial license varies by license type. Fees are non-refundable.

  4. There are opportunities throughout the application to upload supporting documentation for name changes, any "YES" answers to character and fitness questions, and if you are a veteran currently serving in the military or a military spouse. Be sure to have this documentation available before you begin.

  5. You will need to provide supplemental forms. There will be download links for "Verification of Education" and possibly for "Verification of Other State Licenses". The Board will make every effort to verify licensure in other states and any national certification requirements. If Board cannot verify, you will be required to provide the needed documents.

  6. The Board reviews applications in order of receipt. If your application is missing information, you will be contacted by email within 10-15 business days. The email will list the information that is required to complete your application. You have 60 days to submit the required information. If you do not submit the required information within 60 days of the receipt of the email, your application will be closed, and a new application and a full (non-refundable) application fee will be required.
