Application For Reinstatement of Allied Health License

Things to Know Before You Begin

  1. Did you get fingerprinted? A criminal history records check is required by law in Maryland. Completing your background check BEFORE you begin will expedite the licensure process. Click CHRC Instructions for details.

  2. Do you have enough CE credits? Practitioners must upload proof of continuing education credits in a 2-year period preceding the date of the submission of the reinstatement application. Please have this documentation available before you begin this application. For information about the type of CE the Board recognizes and the required documentation, please visit Continuing Education.

  3. Who may use the online reinstatement application? A practitioner whose license has expired for any reason may reinstate their license using the online reinstatement application.

  4. Who may NOT use the online reinstatement application?
    1. A practitioner whose license has been revoked, surrendered, or suspended may not use the online application to reinstate. Contact felicia.wright@maryland.gov to request a paper application.

  5. The fee to reinstate a practitioner license varies by practitioner type. Please visit the Reinstatement Fees page.

  6. There are opportunities throughout the application to upload supporting documentation. Upload links are provided for things like name changes, CE certificates, and any "YES" answers to character and fitness questions. Be sure to have this documentation available before you begin.

  7. The Board reviews applications in order of receipt. If your application is missing information, you will be contacted by email within 10-15 days. The email will list the information that is required to complete your application. You have 60 days to submit the required information. If you do not submit the required information within 60 days of the receipt of the email, your application will be closed, and a new application and full (non-refundable) reinstatement fee will be required.
