You also have the option of mailing a check or having your employer or 3rd party mail in payment for you. When you finish, you will receive your invoice to print out. Please be sure to mail your invoice along with your check or give a copy to the payer to mail with the check.

 
   
Logon is easy and secure. Simply use your medical license number, date of birth, and last four digits of your social security number as your password.  
   
You may complete part of the application, leave it, and return later to finish. All your work is saved and you return exactly where you left off.   
   
After you complete your application you may log back on at any time to review and print out the application and a copy of your receipt.
