Summary:
Reprimand. Probation for a minimum of eighteen months; terms and conditions. The practitioner failed to meet appropriate standards of care, and failed to keep adequate medical records, as determined by peer review. For a minimum of six months, the practitioner is prohibited from prescribing or dispensing all CDS, except for in emergency cases. In emergency cases, the practitioner may not issue more than one prescription for each patient during the CDS prohibition period, and the prescription may not exceed the lowest effective dose and quantity needed for a duration of five days.
