name and contact information. A confirmation of this transaction will be emailed to you as the requester.
Verify and confirm your selections.
Proceed to checkout to pay by credit card: Visa, Mastercard, or Discover ($50 for physician license verification or $25 for allied health practitioner verification).
An encrypted, digitally certified verification letter is emailed immediately after the transaction is completed.

Important:
You cannot use this online verification system to request verification of an unlicensed medical practitioner (registration prefix "P"). Contact the unlicensed medical practitioner's training facility to obtain a verification of licensure.
