Guidelines for Completing the Application

Before You Begin

  1. Did you get fingerprinted? A criminal history records check is required by law in Maryland. Completing your background check before you begin will expedite the licensure process.
  2. Have you submitted your UA and FCVS to Maryland? The UA and a completed FCVS profile are required to use the online program for Initial Medical Licensure in Maryland. Establishing an FCVS profile can take several months and requires contacting FCVS, an organization independent of the medical board. Applicants are encouraged to complete their FCVS applications six months before they need their FCVS profile submitted to the state medical board for licensure.
    The Maryland application requires the applicant to supply the Federation ID (FID) number obtained from the applicant’s UA account . Data from the UA will be imported into the IML application.
  3. Who can use the online application? This online application is restricted to physician applicants who have never been issued a Maryland medical license.
  4. Can I upload documents through the application? There are opportunities throughout the application to upload supporting documentation, such as name change documents, supporting documents for any “yes” answers to the character and fitness questions, and supporting documents for service members, veterans, or military spouses. Be sure to have this documentation available before you begin.
  5. What information do I need? Applicants using the online application must account for all time since medical school graduation, including non-working time, postgraduate training, and employment. Having this information on hand before starting the application is highly recommended.
  6. What are the fees?
    1. Application processing fee of $310.
    2. Prorated $20 monthly licensure fee assessed from the month of license application approval to the month of license expiration. The prorated licensure fee will not exceed $480. COMAR 10.32.01.12
    3. Physician licenses expire on September 30 of an even or odd year based on the applicant’s last name. If your last name begins with A–L, your license will expire on September 30 of the first even year following issuance. If your last name begins with M–Z, your license will expire on September 30 of the first odd year following issuance. The Board issues a license once the applicant has met the requirements for licensure, which means your physician license could expire the same year it was issued.
  7. When will the Board review my application and what happens if it is incomplete? The Board reviews applications in order of receipt. If your application is incomplete, Board staff will email you about the deficiencies in your application. You have 60 days to correct the deficiency. If you fail to correct the deficiency within 60 days of receiving the email, the Board will close your application, and you will be required to submit a new application and pay the non-refundable application fee.
