Practitioner Complaint Form

The Maryland Board of Physicians (the Board) is responsible for investigating complaints against licensees and for taking action against the license of those who fail to maintain Maryland's high standards of medical care delivery or who break the laws governing licensure.

 Before You Begin
General Complaint Information
  • There are statute of limitations with respect to filing a complaint. However, there is no statute of limitation for the following scenarios:
    1. Criminal convictions, sexual misconduct, or similar boundary violations;
    2. Reciprocal discipline matters;
    3. Ongoing substance abuse;
    4. Fraudulent concealment of material information;
    5. Acts that occur while patient is a minor.
    The Board is generally best able to obtain accurate and complete information when a complaint is submitted as soon as possible after the matter that prompted the complaint.
  • Copies of medical records may be obtained as part of the investigation process. Keep in mind that medical records have to be kept for a minimum of 7 years. For more information, visit Medical Record Retention & Fees FAQs .
  • Once a complaint is submitted to the Board, it cannot be withdrawn.
  • The Board does not have the authority to order a licensee to provide money or provide other personal remedies to an individual who thinks he/she has been harmed by a licensee. This type of complaint is pursued by contacting an attorney and initiating a suit in the civil justice system. Even if the Board disciplines a licensee, that information is not admissible in a civil action, even though the disciplinary action may be based on the same facts.
Information You May Need to Gather
  • You will need the office phone number of the practitioner. Please be prepared to provide this before you start.
  • If the complaint involves a specific patient, please be sure to include the patient's full name and date of birth, names of any hospitals or health care facilities where care was provided and specific date(s) of all appointments and consultations referenced in the complaint.
  • You will have an opportunity to upload supporting documents (medical records, copies of emails, photos, etc.), so have these ready.

Note:  If you started a complaint but did not finish, you may sign in. You will need your Complaint ID and email address. Anonymous complaint submissions cannot continue complaints or check the status of their complaint.
