Announcements

Practitioners who prescribe or dispense CDS must be registered with the Prescription Drug Monitoring Program (PDMP) in order to obtain a new or renewed CDS license. Practitioners that have already registered with the PDMP do not need to do anything additional. For more information, click PDMP Registration FAQs or visit the PDMP/Crisp Registration website.

Criminal History Records Checks are only required for new licensees and reinstatements--not for renewals.

7/08/2024 Correction to Information About the CME Requirement in the 2024 Biennial License Renewal Postcard:
The 2024 biennial license renewal postcard to physicians incorrectly stated that licensees applying for their first license renewal after initial licensure or reinstatement are exempt from the CME requirement. The postcard should have read: Licensees applying for their first license renewal after initial licensure are exempt from the CME requirement.

Please note that licensees applying for their first license renewal after reinstatement are not exempt, and must attest in their application that they have satisfied the CME requirement by the expiration date of the license.

License Renewal Period: 7/15/2024 To 9/30/2024*
