Announcements

Practitioners who prescribe or dispense CDS must be registered with the Prescription Drug Monitoring Program (PDMP) in order to obtain a new or renewed CDS license. Practitioners who have already registered with the PDMP do not need to do anything additional. Criminal History Records Checks are only required for new licensees and reinstatements, not for renewals.

Are you planning on retiring? To change your license status, apply for Inactive Status (opens in a new window) or Emeritus Status (opens in a new window) to apply now. For more information on Emeritus Status, visit the FAQ page (opens in a new window) .

Volunteer physicians must submit a new Exemption From License Fee (opens in a new window) application to the Board for approval to renew as a volunteer.

For licensees whose last name begins with M-Z and expires 9/30/2025.
