License Copies FAQs


Anyone who is newly licensed, reinstated, or has renewed their license can log in to the Practitioner Profile and download a digital copy of their license. Log in to your Practitioner Profile. Click on “Print Duplicate License” in Portal Navigation.

If you are newly licensed, set up your Practitioner Profile by clicking “New Registration” on the login page. After your Practitioner Profile has been set up, you can obtain a copy of your license by printing it from your Practitioner Profile.

You will have the option to print out a copy of your license during the renewal process. You may also obtain a copy of your license by downloading and printing it from your Practitioner Profile, or you may obtain a copy of your renewed license directly from the Board’s website.

No, the Board will not mail hard copies of a license.

Yes, you can obtain a duplicate license by printing it from your Practitioner Profile if your license is on Probation status, if you were licensed through conceded eminence, or if you have a volunteer license. The process is not different.

A copy of your license will indicate that the practitioner is licensed through conceded eminence and will state the location at which the license is valid.

A copy of your license will state that the license is a volunteer license.
